Diagnosis of IGF2-secreting Tumour The diagnosis of tumour-induced hypoglycaemia should be suspected whenever nonketotic fasting hypoglycaemia is found in association with suppressed levels of plasma insulin and C-peptide, and should be confirmed by the finding of an abnormally high IGF2 to IGFl ratio due to normal or high plasma IGF2 levels and very low plasma IGFl levels. Since tumours that cause hypoglycaemia are generally large or very large by the time they present with hypoglycaemia, localisation rarely presents a problem, though in some cases it is necessary to resort to the most modern imaging techniques.
